EHS Soccer Parents Information Tryout Information FEES (Includes the purchase of an Eagles Pass): 65.00 Eagles Pass Fee-Checks payable to Eastern High School The Eagles Pass includes Admission to all regular season home athletic events A limited editions Eastern Eagle T-shirt Try out fee Insurance Fee *Trainer Fee (This pass is for student athletes only) $200.00 Booster Club Fee – Checks payable to EHS Soccer This will be collected prior to try outs and returned if you player does not participate in EHS soccer this fall. Players and parents will also need to watch the Health and Safety Video on the following JCPS website: http://jcps.jefferson.k12.ky.us/athletics/default.asp The form for the Health and Safety Video is also below: EHS Player Information Form 2010-2011 Authorization to Treat Minor Form Health and Sports Safety Video Signature Combo Form Physical Form Physical Addendum and Parent Permission form		KHSAA Health and Safety Video IMPORTANT INFORMATION, PLEASE READ Starting this year the KHSAA is requiring that ALL players and a parent must watch a Health and Safety video. Each player and a parent must sign a form stating they have seen the video BEFORE the player may participate in any KHSAA sports - no exceptions. The Health and Safety Video is now available to be viewed online. Click on the following: http://jcps.jefferson.k12.ky.us/athletics/default.asp Click on: High Schools then Health and Safety Video. After viewing the video print the Health and Safety Video Signature Form and sign it. Both the parent and player will need to sign a form. Bring the signed forms to the coaches.
